- Rutgers Cooperative Extension Program Evaluation - Youth Audience (110k PDF)
This form is aimed at youth in Grades K-3. The PDF version can be used for a wide variety of topics and experiences and has fill-in fields for topic, date, and presenter.

- Rutgers Cooperative Extension Program Evaluation - Older Youth Audience
There are two teaching evaluation forms for older youth audiences; GR4-13A_FORM (89k PDF) is a post program evaluation that provides an opportunity for the participant to list things they learned and/or ways to use information learned. GR4-13B_FORM (100k PDF) is a retrospective pre/post evaluation that has three fill-in fields for the presenter to add in topic-specific statements/questions and fields for entering program title, date, and presenter.

This form is designed for youth in grades 4 and higher participating in judged 4-H presentation events or programs that include a formal public presentation as part of the project. The evaluation form and corresponding Excel worksheet have received IRB approval and cannot be altered in any form.
