There are three generic teaching evaluation forms for youth audiences and one designed specifically to evaluate 4-H Public Presentations.   1. Rutgers Cooperative Extension Program Evaluation - Youth Audience (110k PDF)
    This form is aimed at youth in Grades K-3. The PDF version can be used for a wide variety of topics and experiences and has fill-in fields for topic, date, and presenter.

    This Excel worksheet is for entering data and calculating results from the Youth Audience/GRK-3 Program Evaluation form ONLY. You can download and save a blank version, then save as a new file for each session taught. Directions are included in the worksheet or as a PDF file.

  2. Rutgers Cooperative Extension Program Evaluation - Older Youth Audience
    There are two teaching evaluation forms for older youth audiences; GR4-13A_FORM (89k PDF) is a post program evaluation that provides an opportunity for the participant to list things they learned and/or ways to use information learned. GR4-13B_FORM (100k PDF) is a retrospective pre/post evaluation that has three fill-in fields for the presenter to add in topic-specific statements/questions and fields for entering program title, date, and presenter.

  3. New Jersey 4-H Public Presentations County/State Evaluation (101k PDF)
    This form is designed for youth in grades 4 and higher participating in judged 4-H presentation events or programs that include a formal public presentation as part of the project. The evaluation form and corresponding Excel worksheet have received IRB approval and cannot be altered in any form.

    This Excel worksheet is for entering data and calculating results from the NJ 4-H Public Presentations Evaluation form ONLY. After you download the file, save a blank version and use a new one for each educational event. Up to 200 participants can be entered for a session. Results are calculated automatically once data is entered.
